As libraries are embracing all that Web 2.0 has to offer, many library wikis have sprouted up on the web.

Lamar Guides
Lamar University's Mary and John Gray Library offers links to various library pathfinders for faculty, staff and students. Research guides, Subjects page, and Database page are all well done and fairly user-friendly, with links to "Quick Guides" for using the library resources. You have to click on "Show all pages" to find these options.
PAMS Reference
North Carolina State University's Physical and Mathematical Sciences Library wiki, with resources listes by subject, resource formats, courses, etc.
Ohio University BizWiki
"A collection of business information resources...designed to assist business researchers in finding the best resources for their projects or topics."
PALS RefWiki
A "is a reference tool for members of the Prairie Area Library System to use and build." This site is a work-in-progress. The layout is good, but not all categories contain infomation.
Best Practices Wikis
